Abstract: 

Introduction: Fixation and preparation of the cadavers for anatomical dissection of the different human organs has been routinly practiced in many medical institutes. Chemicals used for Fixation, usually caused too many artifacts including abnormal histological appearance of different organs, unpleasant smell, unfixes materials, etc... .To stablish new method to fix cadavers in order to avoid at least part of above problems. Materials & Methods: Ten cadavers were used and divided into experimental and control groups. Each experimental group received 12 liters of new fixative contaning (Borax Buffer, HCHO; absolute Ethanol; Boric acid; Ethylen glycal; Lisoformin;, potassium nitrate; salicylic acid; H202 and thymol). Control receive regular codaver fixative. The new fixative fluid in all specimens, were injected gradually in the period of 48 hours, and kept in solution of 5% sterillium or phenol and ethylen glycol for 14 months. Results: In comparison with the controls, the results showed excellent Fixation in all anatomical structures with least artifacts in all examined experimental groups. Conclusion: It is concluded that using new fixator increased that using new fixator increased stiffness and quality of strate muscular tissue, improved fitation of fatty tissue, fasciae and membranes. By using this new Fixation. priphral and central nerves and blood vessels were better distinguished than routhine fixator.

Abstract: 

Background: Fractures of tibia and fibula are one of the most common fractures of extremities in traumatic patients. Treatment of these fractures usually is surgical Fixation of the tibia but there is not plenary study about the Fixation of the fibula.Methods: All patients refers to Rasool hospital from Oct.2009 to Oct.2011 that achieved including criteria were entered in this prospective education and divided in to two groups: group 1 with 31 patients that was treated with Fixation of the tibia and the fibula and group 2 with 34 patients that was treated with Fixation of the tibia without Fixation of the fibula and then severity of pain after the surgery, time of union, complication of the treatment and duration of the surgery were compared between them.Results: In group 1 severity of the pain and time of the union were significantly less than group 2. No nonunion was observed in group 1 but one was seen in group 2 that finally required surgical management. Duration of the surgery in group 1 was significantly longer than group 2.Conclusion: Although Fixation of the fibula has taken longer duration of surgery, have better clinical outcomes.

Abstract: 

Background: Clavicle fracture, especially midshaft fractures is a common bone injury that happens after a fall onto the shoulder. It is uncommon to need surgical treatment. We would like to report on a selected group of cases that required open reduction and plate Fixation.Methods: 53 consecutive adult patients with severely displaced fresh fractures of the middle third of the clavicle, who were treated by open reduction and plate internal Fixation, were studied in a one year period in Zahedan, Iran. Union shoulder motion and complications were analyzed in 49 patients (29 women, 20 men) who referred for follow-up, with an average follow-up of 12 months.Results: The mean age of patients was 33.4 years (19-62 years). 48 cases (97.9%) obtained union and one case (2%) developed malunion and required corrective osteotomy. Dermatologic complications were observed in 2 cases. All the patients had full motion and function.Conclusion: Open reduction and plate Fixation of severely displacement fresh clavicle fracture in well-selected cases can result into good union with minimal complications rate.

Abstract: 

Background: The anterior cruciate ligament is the primary structure that controls anterior displacement in the unloaded knee.Anterior cruciate ligament reconstruction may return normal functions of the knee. The aim of the present study was to compare three different femoral Fixation techniques in anterior cruciate ligament reconstruction.Methods: In a clinical trial study, 120 patients that were candidates for ACL reconstraution were randomly divided into 3 groups of 40 individuals. The femoral Fixations were by the three methods of "Aperfix", "Rigidfix" or "Endobutton". The cases were re-examined 12 months after surgery and evaluated by Lysholm score as well as with KT-1000 machine. The results were compered together for the three above methods.Results: The Lysholm score showed improvement from pre-operative values in all three techniques: From 63.21±18.59 to 90.64±9.47 in "Endobutton" group, from 65.72±18.74 to 96.22±5.35 in "Aperfix" and from 69.21±17.45 to 90.64±9.47 in the "Rigifix" group. There were 6 failures in "Endobutton", 4 in "Rigidfix" and one in "Aperfix" group. Theanterior displacement tibiawas 3.96±1.58 millimeters in "Endobutton", 4.28±1.48 in "Rigidfix", and 4.03±1.79 millimeters in "Aperfix" group. There was no significant difference in the operating time in the 3 groups.Conclusion: Regarding the instant stability of the graft, the "Aperfix" method seemed stranger. Further investigations with larger number of cases and longer duration of follow up are recommended.

Abstract: 

Background: Ankle syndesmosis injury is among the common ankle injuries. This injury results in a long-term inability to perform physical activities. Various surgical techniques have been experimented that each accompanies with limitations, advantages, and disadvantages, and controversial outcomes have been presented. In the current study, two techniques of tightrope and tricortial-screw Fixation were compared. Methods: The current randomized clinical trial study was conducted on 30 patients with ankle syndesmosis injury in the Alzahra and Kashani hospitals, Isfahan, Iran, during the years 2014-17. The patients were randomly divided into two groups of tightrope and tricortical-screw treatments. The two groups were followed and compared considering the range of motion, American Orthopedic Foot and Ankle Society (AOFAS) score, required time for return to work, and need for reoperation within 6, 12, and 18 months. Findings: Patients of the two groups were not different regarding age, gender, and body mass index (BMI) (P > 0. 050); but the recovery time and need for reoperation were significantly less in tightrope group (P < 0. 001). According to AOFAS score, tightrope group was better considering pain, activity limitation, maximum walk, and walk surface (P < 0. 050); but they were not statistically different regarding plantar and dorsiflexion, gait, sagittal motion, hind foot motion, and function (P > 0. 050). Conclusion: Based on the findings of the current study, tightrope Fixation technique was accompanied with better outcomes in comparison to tricortical-screw Fixation in ankle syndesmosis injury.
